Reporting Procedures and Guidelines

Please review the information on this page before submitting a report to gain a clearer understanding of the Concerns Hotline, the Audit Committee’s role, and the reporting process.

Board Policies to Consider  

The State Board for Community Colleges and Occupational Education has issued two policies you should be aware of.

What Happens to the Information I Submit?

  • Human Resources issues are directed to the Human Resources Department at the System Office.
  • All other matters are reported to the Director of Internal Audit.
  • Items are reviewed in summary by the Audit Committee, a subcommittee of the State Board for Community Colleges and Occupational Education, including the resolution of the issue.  
  • After you complete your report, you will be assigned a unique code called a “report key.”
  • Write down your report key and password and keep them in a safe place.
  • Use your report key and password to check your report for feedback or questions often.
  • Your submission is reviewed to determine whether sufficient information exists to conduct an investigation.
  • Using the instructions on this webpage we can communicate with you anonymously.
  • Investigations or reviews can only occur when sufficient information is presented. It is important to provide as much detail as possible.
  • Make sure to log back in periodically to check the status of your issue and provide any additional requested information.
  • The concerns hotline allows you to upload documents, photos, and other relevant information for review.
  • Due to the confidential or sensitive nature of most of the reports received, CCCS is generally unable to provide reporters with the results of any review or investigation.
  • We will notify you via the concerns hotline when the review has concluded.

Anonymity  

You may remain anonymous by not reporting your personal information.

The hotline helps promote confidentiality and provides safeguards against retaliation. 

When reporting anonymously, be sure to provide as much information as possible and follow-up on your report regularly so that CCCS can fully review and investigate your concerns.

  • Call 888-299-5440 toll-free within the U.S., Guam, Puerto Rico and Canada, or
  • Submit a report online by selecting the appropriate campus from the list below.

EthicsPoint is NOT a 911 or Emergency Service

Do not use EthicsPoint to report events presenting an immediate threat to life or property. Reports submitted through EthicsPoint may not receive an immediate response. If you require emergency assistance, please contact your local authorities.
